From Jacksonville.com:

The Florida Department of Highway Safety and Motor Vehicles now offers those involved in minor traffic accidents an online portal designed specifically for drivers involved in crashes that do not warrant a police report. The online reporting form is available at www.flhsmv.gov/fhp/misc/CrashReport. This online form was designed to facilitate the process of submitting required accident information directly to the Department of Highway Safety and Motor Vehicles.

Accidents that not involve injuries or a tow truck, cause damage totaling less than $500 and are not considered “hit and run” are eligible to be reported on the new website. In fact, drivers involved in such accidents are required to submit a written report to the state within 10 days of the crash.

The online questionnaire easily guides users through each step the report. Completed reports are automatically submitted to the state, and may be saved and printed.
